The contact form collects the name, email address, and any optional phone number, city, child age range, preferred contact method and message you choose to provide. It is deliberately a general inquiry form and does not ask for diagnoses, medical history, medication details, health-card numbers, school details or Ontario Autism Program registration numbers.
How it is used
Information submitted through the contact form is used to respond to your inquiry and to discuss whether Therapy Ahead services may suit your family. Detailed clinical information is gathered later, through an appropriate intake process, not through this website.
